Your AristaMD Care Coordinator can help set up visits, coordinate necessary testing, and work closely with local in-network providers for follow-up care.
Will my primary care doctor know what’s happening?
Yes. AristaMD updates your primary care doctor so that everyone stays informed.
Before Your Appointment
Bring Your ID and Medications
Bring your ID and a list of your medicines or the bottles. Include vitamins and supplements.
Write Down Symptoms and Questions
Write down any changes in how you feel and any questions you want to ask. Even small changes matter.
Have Recent Health Info Ready
If available, have recent readings like blood pressure, pulse, height, and weight.
Invite a Family Member

Let your care team know if a caregiver will attend your appointment so they can help make arrangements.

How does virtual heart care work?
Our Care Coordinator team will work with you to help with scheduling your first appointment. From there, you’ll meet with a heart doctor virtually, using secure video visits. If you need additional testing or procedures, we work with local testing facilities and cardiologists so that you get appropriate follow-up care in person. After each virtual visit, our providers share information directly with your primary care doctor so everyone understands your care plan and health goals.
How do I know if I qualify?
This program is currently for Original Medicare members but stay tuned for more updates about other in-network plans.
