Our approach is fueled by a robust, data-driven methodology focused on operational excellence and a personalized, focused approach that makes a difference. This translates into more efficient provider engagement and, most importantly, healthier patient lives.

I always like to mention that we believe it’s critical to review data thoroughly before contacting a provider. We don’t want to bother them unnecessarily. We approach them with a clear, determined plan, not just a general request to review a list.

We coordinate closely with care teams on coding, exclusions, pharmacy records (as Kate mentioned), and appropriate chart documentation. We understand the administrative burden providers face. By working with the care team first, we ensure the patient list is correct before the cardiologist and provider discuss specific cases.

We form relationships and follow up with organizations and clinics. Our team becomes personally invested; we can see the impact in the data as gaps close. Once educated, clinics often come to us for advice on the next measurement year or for tricky situations where a patient isn’t falling off the list. We provide advice, and if it’s clinical, we connect the AristaMD cardiologist to the provider via eConsult or other asynchronous methods that fit the provider’s busy schedule.

Our goal is to make the burden lighter. We track and update patient records. As Kate and I were saying, we will monitor a patient indefinitely to ensure the gap remains closed. We check in to confirm that a patient like Maria, who successfully adhered to her medication last year, hasn’t run into new problems. We let the provider or care team know if she unexpectedly pops up on the list again.
