What Is This Program?

The ACCESS Model is a new Medicare initiative from the Centers for Medicare & Medicaid Services (CMS). It was created to give people with chronic conditions more day-to-day support — not just during appointments, but in between them, where most of life actually happens.

AristaMD is a participating provider in the ACCESS Model. Through ACCESS, you get a care team that checks in regularly, helps you track what matters (like blood pressure, weight, and lab numbers), and works alongside your existing doctors to help you reach your health goals.

This is care designed to fit into your life — by phone, video, or connected devices.

FAQs

How do I know if I qualify?

This program is for Original Medicare members who need additional support managing heart and kidney care.

Is this a replacement for my doctor?

No. We work alongside your existing doctors. We help coordinate your care and support you between visits. We don’t replace your primary care provider or specialists.

Will this change my Medicare coverage?

No. You keep your Original Medicare and your regular benefits, and you can still see any provider who accepts Medicare.

Do I have to use technology I'm not comfortable with?

No. This program is designed to work for you, whether or not you’re comfortable with technology. Most check-ins can happen by phone, and any devices we provide (like a blood pressure cuff or scale) are simple to use, with step-by-step instructions if you ever get stuck.

Can I leave the program if I change my mind?

Yes. Taking part is voluntary, and you can stop at any time, for any reason, with no effect on your regular Medicare benefits or your relationship with your doctor.

What are some reasons I might not be able to use this care option?
  • You don’t meet the eligibility requirements for the condition you want support for.  
  • You’re in a Medicare Advantage Plan.
  • You’re part of a small, randomly selected, control group. Because this is a new care option, Medicare is randomly assigning a small number of applicants to a control group.This helps Medicare evaluate the new care option and has nothing to do with you or your health. Your normal Medicare coverage, benefits, and rights are not affected in any way. If you’re randomly assigned to this group, you won’t be able to use this care option for 12 months. If you have questions, you can call 1-800-MEDICARE (1-800-633-4227). TTY users can call 1-877-486-2048.
What happens after I sign up?

AristaMD will confirm your eligibility and enroll you in the program. We’ll share your care plan with your primary care provider to ensure your care is coordinated and you’ll start getting care and support that’s personalized for the condition(s) you need help managing.
